On this motor there is a crossover tube and check valve for the EGR system mounted on the back of the motor which goes from head to head and the valve is in the middle behind the intake manifold, often either the valve or the tube can rust out and give you a massive exhaust leak that thunders and rumbles in the cab. Check for exhaust leak behind the engine and if there is follow the tube down to each head, it is retained by one bolt on each head (tight to get to! ) remove these bolts and the hose attached to the valve and get the assembly out where you can work on it. If the valve is bad, soak the thread area with penetrating fluid and leave soak for an hour or so before attempting to remove it, these get extremely baked and rusted on, then place the base of the mount in a vise and remove the valve ( often attempting to remove the valve while the tube is on the vehicle will result in the the tube folding over and pinching itself off). If the tube is rotted out replace it and the valve as well ( the tubes are expensive but the valves are cheap LOL).

could be try reaching back with your had and not touch the engine and see if you feel exhaust blowing out and then find out exactly where. It also could be the intake manifold leak but most likely is a broken egr valve pipe pretty common.
